It's a boxed warning, the FDA's strongest. Tendons can tear months after the last pill.
“Tendinitis or tendon rupture can occur, within hours or days of starting CIPRO, or as long as several months after completion of fluoroquinolone therapy.”
Stop and call your doctor at the first sign of tendon pain or swelling. The risk is higher if you're over 60 or take steroids.
